Follistatin is a single-chain gonadal protein that plays a crucial role in inhibiting the release of follicle-stimulating hormone (FSH). It is encoded by the FST gene, which produces two isoforms—FST317 and FST344—comprising 317 and 344 amino acids, respectively, due to alternative splicing of the precursor mRNA. Follistatin binds directly to activin, acting as an antagonist and specifically inhibiting the production and secretion of pituitary FSH.

Research is being conducted on follistatin's involvement in muscle growth regulation in mice, where it serves as an antagonist to myostatin (GDF-8), a member of the TGF superfamily that restricts excessive muscle development. Lee & McPherron found that eliminating GDF-8 genetically (through knockout mice) or increasing follistatin levels significantly boosted muscle mass. Furthermore, studies in 2009 involving macaque monkeys showed that gene therapy to regulate follistatin also led to muscle growth and enhanced strength.
